Register for Patient Portal
Prior to your appointment, our Patient Wellness Team Member will send you an email with a link to you Patient Portal needed to complete the Health History questionnaire, HIPPA/Privacy, Insurance Practice Form and consent forms for telemedicine and weight loss management.
Use the link Virtual Visit
Before your appointment, sign into your Patient Portal to obtain your access link to be used at the time of your appointment. You will have thirty minutes with your provider to discuss your medical history, health risks, and goals. Your customized wellness plan will be reviewed along with recommendation for supplements, a nutrition plan and special lab work.
Receive your medications
Your weight loss medication kit will be delivered to your home in 5-7 days.
Follow-up
Your Patient Wellness Team Member will reach out to you to schedule your follow-up virtual consult. Follow-up will be scheduled 4 weeks after your initial consult.
Labwork
Patients are required to submit routine labs completed within 6 months of the initial virtual visit. Otherwise, our Integrative Healthcare Provider will email a lab order to be completed prior to the delivery of the medication kit.
